Herts Community NHS Trust: Understanding the Year 6 Online Health Questionnaire
Project Brief
Herts Community NHS Trust wanted our help to create an animation explaining the Year 6 Online Health Questionnaire to children and their families.
The Lancaster Model is an online health assessment questionnaire designed to proactively identify the physical, emotional, and social health needs of Year 6 children as they prepare for the transition to secondary school. It ensures the NHS can catch any health or wellbeing issues early and offer direct support, aligning with national guidelines like The Healthy Child Programme.
Creating the animation
Communicating healthcare information to both children and their parents requires real sensitivity and clarity. The animation was created to demystify the questionnaire process explaining exactly what school nurses do, reassuring children that it is a safe and supportive space to talk openly about their feelings and health and clarifying data protection rules for parents.
Our artist, Giulia, brought it to life through warm, engaging whiteboard animation, making what could feel like an unfamiliar process feel approachable and welcoming for children and families alike.
